Claim Procedure
Step 1: Prepare the information or documents
Policy number
An 10/11-digit alphanumeric combination that you may find in your policy
- Referral letter
- Medical report
You may need to prepare the following information/documents according to your claim.
Medical report/receipt/discharge slip
Showing the date & time of admission and discharge
Attending physician's statement
Please make sure your attending physician completed statement during admission
Medical/payment receipt
Other
Other relevant document(s)
Step 2: Complete the online claim form (except Group Medical)
- Your contact details
- Claimant details
- Select the claim type and fill in the relevant information
- Your bank account details
- Upload supporting documents

Step 3: Submit
Upon completion, you will be assigned a tracking number. We will contact you shortly afterwards. You will also receive an email notification from us.
Step 4: Check claims status online
You can easily check your claims status anytime, anywhere, ensuring you stay informed throughout the process.






Step 5: Direct bank-in to your bank account
We will direct bank-in to your bank account once the submitted claim form has been approved.
