Claims Procedure:

Please complete the claim form, and submit together with all relevant documents to us within 30 days from the date of accident/ consultation (for medical claim) or after discharge from hospital (for hospitalisation claim) to our Claims Department: 9/F, Cityplaza One 1111 King’s Road Taikoo Shing, Hong Kong or Avenida Da Praia Grande, No. 693 Edif. Tai Wah 13 Andar A&B, Macau

 

To facilitate the processing of your claim, documentary evidence is very important. Following are examples of documentary evidence:

 

For Outpatient / Medical Claim:

  • Original medical reports / medical receipts with information as listed:
    • Treatment date
    • Name of patient
    • Diagnosis
    • Attending physician’s stamp and signature

For In-patient / Hospitalisation Claim:

  • Original medical report
  • Laboratory report
  • Referral Letter
  • Discharge summary / Discharge slip
  • Original bills and receipts for claim expenses with information as listed:
    • Treatment date
    • Name of patient
    • Diagnosis
    • Attending physician’s stamp and signature

Important Notes:

  1. Incomplete claim form cannot be accepted for processing of claim. Please attach original copies of all relevant documents.
  2. Please send copy of the payment document and breakdown if other insurance company has already paid part of the medical expenses.
  3. All medical reports, information and evidences as required by us shall be furnished at the Claimant’s own expenses.
  4. Further information may be needed.
