Critical Illness & Hospital Claim 
Outpatient Claim 
   

Claims Procedure:

  1. Please submit your claim within 30 days from the date after discharge from hospital/ of consultation.
  2. The Policyholder and the Insured Person and/or his/her legal representatives must complete all questions in Part I of claim form and sign on it.
  3. The attending Physician must complete all questions in Part II of claim form, rubber stamp, date and sign on it.
  4. Send the complete claim form, together with all relevant documents to:
    MSIG Insurance (Hong Kong) Limited
    Claims Department
    9/F, Cityplaza One
    1111 King’s Road
    Taikoo Shing, Hong Kong

To facilitate the processing of your claim, please submit the following supporting documents:

  • Original medical reports
  • Laboratory reports
  • Discharge summary
  • Bills and receipts for claim expenses with information as listed:
    • Treatment date
    • Name of patient
    • Diagnosis
    • Attending physician’s stamp and signature

Important Notes:

  1. The commencement date of the illness has to be within the effective period of policy coverage.
  2. Pre-existing conditions are not covered in our Policy unless specified in the policy.
  3. Original receipts will only be returned upon request.
  4. Please send copy of the payment document if other insurance company has already paid part of the medical expenses.
  5. All medical reports, information and evidences as required by us shall be furnished at the Claimant’s own expenses.
  6. Incomplete claim form cannot be accepted for processing of claim. Please attach original copies of all relevant documents.
  7. Further information may be needed.
